The attached form contains medical information relevant to the fertility status. Please fill as many items as you like. Don't forget to fill in the Email field under General Information so we could send you our recommendation.

The information may allow for general recommendation concerning additional workup or treatment. These recommendations should be regarded as informal medical opinion. Formal recommendations can only be given following personal medical interview and exam.

Information sent to this site will be kept confidential, though one should be aware of the fact that like every email message, email sent to this site may be intercepted by unauthorized persons.
